Community care is an option when the VA cannot provide the required services due to specific eligibility factors, availability, or individual circumstances.

Availability of VA Care: If the required care is not available at a nearby VA facility, the Veteran may be referred to a qualified community provider.

Follow this process to request care from White Chiropractic. We are an in-network Community Care Provider.
Contact White Chiropractic. We will contact the VA on your behalf to let them know that you are interested in treating with Dr. White throught the community care program. Call us at 704-799-1416
Get a referral from your VA health care team. Ask your VA health care team for a referral to White Chiropractic, a community provider. They’ll review your request to make sure you’re eligible. Then They’ll contact you to confirm that you want to get community care and the type of appointment you need. They’ll then prepare your referral.
Schedule your appointment. After you have your referral, you can schedule your appointment at White Chiropractic . White Chiropractic ususally receives a copy of the referral as well so we will contact you once we receive the referral in order to schedule an appointment, if you have not already done so.
Go to your appointment. They’ll share your medical records with White Chiropractic. But be sure to bring copies of any images (like CT scans or MRIs) that the provider asks you to bring with you.
Keep track of ongoing appointments and referrals. If they approve you for continued care, you can schedule follow-up appointments with White Chiropractic. Keep track of how many appointments you go to and how long you’re approved to get care. If you need more care, you or White Chiropractic can request a new referral.
